Black Belt Application Form

The Student filling out this form must be an Adult of consenting age as defined in their local laws. If you are not an Adult as described above you must seek the support of a consenting Adult to complete this form for you.

Notes on Fees: Testing fees are non refundable once the applicant above attends their first Black Belt testing.

Notes on Eligibility: DBNs may apply with the permission of their school owner, black belts must have both the permission of their school owner and have held their current black belt rank for a minimum of 2 years. All application forms will be sent to your instructor for their approval before being accepted.

Notes on promotion: Your school owner has the final decision on your promotion and will submit a formal request to the Han Guk Mu Sool association. Candidates will be required to test at multiple National Testings until their School Owners submits a promotion request to the Association.

Liability: All Candidates entering into this Agreement, hereby waive, discharge, and covenant not to make any claims, charges or suits against Han Guk Mu Sool Ltd and any or all of their successors, assigns employees, owners, officers, instructors and related persons (collectively “releasee”) from all liability to the undersigned or its personal representatives, successors, and assigns for any and all claims, demands, losses or damages and any claims or demands therefore on account of any injury, including but not limited to the death of the participant or damage to property, arising out of, or relating to the Black Belt Testing, even if such a claim arose due the acts, error or omissions of the releasee.
